  • You should use information from a learning log to update other person-centered thinking tools and the person-centered description (support plan).

  • How is what you have learned about a person recorded in your organization? A learning log can replace traditional notes or records to help you to focus on the critical information about the person.
  • How could they make a difference to the people that you support?
  • If you have already use learning logs then how often are they updated and are being used on a regular basis?
